What does transition mean?
Transition means preparing for a change in service.
This may include moving to another service or program.
What does exit mean?
Exit means leaving a MIFWA service.
This may happen because:
- you choose to leave
- your goals have been met
- you are no longer eligible
- funding changes
- you stop engaging with MIFWA
How does MIFWA support transition or exit?
We will:
- provide information in a way you can understand
- support you through transition or exit
- explain how to re-enter services where possible
- use fair and non-discriminatory processes
Planning to leave a service
MIFWA should talk with you about your ongoing support needs.
We may also talk with you about:
- informal supports
- community supports
- a handover to another provider, with your consent
- a letter confirming your exit from service

If you have questions about transition or exit, you can talk with us.
You can:

- ask for information in a way you can understand
- ask someone you trust to help you talk with us
